Required data fields are labeled in red on all name entry screens. Required fields are first and last name, sex, race and date of birth.
Both sex (sex drop down list) and race (race drop down list) fields are drop down lists that contain all valid sex and race codes accepted by the application. Choose the correct codes from these lists.

All available information should be entered. A middle initial or name may make the difference between manual intervention or immediate response. Submitting alias and maiden names may be the difference between a NO RECORD response or a record being returned.
Social security number aids in searches of the databases maintained and searched by DHFS for caregiver and daycare requests.
