Add Request

The mandatory minimum fields are labeled in red as displayed on the data entry screen.  Those mandatory fields are: last name, first name, sex and date of birth.  These fields are not case sensitive. 

Social Security number should be included for caregiver and daycare checks. 

The more information submitted, the more extensive the search and the more precise any match will be.

Options on this page are:

•      Done - returns user to request summary page

•      Add Another Request - displays another blank request screen with fields for name, sex, race, date of birth and social security number

•      Add Alias - displays an alias name screen with name only fields.  There are no additional fees for searches of alias or maiden names

•      Clear form - removes the information entered on the form

•      Cancel - closes the add request screen without saving changes

**Required Fields  Last and First Name, sex and race (even if using unknown) and date of birth.
