Add Alias

When you have keyed in the name, click Add Alias if you need to include aliases or maiden names with the inquiry. You may click Done or Add Another Request to submit more names.

See the alias/maiden name entry display.  You may enter aliases or maiden names for the subject of your inquiry.  The fields will be filled with the name information keyed into the original request.  Type over whichever field need to be changed.  There is no additional fee for an alias name search, however, only the name information may change.  Changes cannot be made on the alias entry screen for sex, race, date of birth or social security number.

Options on this page are:

Done - returns user to the request summary page

Add Another Alias - displays another alias name box for additional aliases or maiden names

Clear form - clears information from alias name form without closing form

Cancel - closes alias name box without saving data entered
